This form has to be signed by the breeder and the vet. The complete form has to be sent to the ALAEU (ingekorevaar@gmail.com).
If a puppy cannot be neutered due to medical reasons: please fill in LATER. As soon as the puppy is neutered on a later date, the postponed desexing form has to be sent in.
Please be aware that no more than 3 puppies can be kept or sold as a breeder, unless you have written approval of the board of the ALAEU.
